Care Navigator - Oncology

The Behavioral Health Care Navigator in Oncology directly supports and promotes the care and social support needs of oncology patients across the continuum of care. This position collaborates with providers, Nurses, Care Managers, Social Workers and others to facilitate seamless continuity of care, social support interventions, and patient engagement to close care gaps, with the goal of assuring superior patient experience and quality outcomes in our Cancer Center. The Behavioral Health Care Navigator connects SMG patients internally and externally to all concrete services during their treatment and beyond. They will regularly assess the needs of the patients and their providers to ensure that all essential referral agencies exist, while also seeking out opportunities for service delivery improvement. They will perform an integral role in clinical data collection, tracking, trending and reporting on all outcomes.

Essential Job functions:

  • Establishes and maintains external relationships with social service agencies and resources by:
    • Promoting ongoing collaboration and regular communications with organizations and facilitators
    • Conducting & documenting routine/weekly outreach calls to patients and providers
    • Demonstrating effective internal and external relationship-building skills

  • Works collaboratively with both internal and external entities to facilitate seamless transitions across the continuum of patient support

  • Manages patients by providing outreach and adhering to an established care pathway and algorithm designed for regular phone or video check-ins

  • Collaborates with behavioral health and manages providing patient information, referrals and assistance inquiries ensuring outreach to patient/family member/caregiver is completed in a timely and efficient manner

  • Maintains a Referral and Concrete Resources Directory

  • Has a daily presence in the treatment room to introduce patients and families to available services and resources

  • Identifies patients and families who would benefit from additional psychosocial support and makes appropriate referrals to the behavioral health

  • Communicates with physicians and other treatment providers as necessary

  • Provides appropriate empathy, warmth and support to patients

  • Proactively reviews and outreaches patients after an elevated score on a distress screening to offer concrete services assistance

General Job functions:

  • Provides support, resources and concrete services to patients and their families during and after cancer treatment

  • Maintains information flow and communications with external support agencies to ensure efficient patient care

  • Demonstrates appropriate and timely use of the EMR

  • Attends all pertinent departmental meetings and trainings that involve behavioral health workflows

  • Assists with special projects as assigned and completes them within the required timelines

  • Effectively communicates concerns/issues to the Manager if Integrative behavioral health appropriately and promptly
